The PMO no longer manages POID changes or requests.
When agencies need to establish, discontinue, or update a Personnel Office Identifier (POI), they must download and fill out OPM Form 1396, then submit the form via email to POI@opm.gov. This form ensures accurate and up-to-date personnel office information for critical functions such as employee surveys and official correspondence.
Agencies are required to complete the form when creating new personnel offices, closing existing ones, or changing key details like mailing address, personnel officer contact information, or survey contact details.

Important: if a POID is created or updated, the agency must notify eOPF so the changes are reflected in our system.
