The eOPF Master Forms List indicates how records should be indexed when a record is determined to be authorized for the Official Personnel Folder (OPF). The Master Forms List and Changes documents are available to download at the end of this article.
Note: Agencies must refer to Ch 3 OPM Guide to Personnel Recordkeeping as the definitive resource to determine if a record is or is not authorized for the OPF. If the personnel record is created from an employment system that is outside of OPM recordkeeping authority, the agency that owns the personnel record must apply the recordkeeping standards as described in the System of Record Notice (SORN) and the Record Schedule for that employment system.
Using the correct form and folder side helps ensure records are accurate, transferable, and compliant with OPM recordkeeping rules.
What is the Master Forms List?
The Master Forms List is the official list of all forms that can be filed in eOPF. For each form, it shows:
The approved form number and name
The document type (such as Personnel Action or Performance Document)
The correct eOPF folder side (Permanent, Temporary, or Performance)
HR Professionals use this list when uploading, indexing, correcting, or reviewing documents in eOPF.
What has changed?
The Changes tab explains what was updated for that quarter. It highlights:
Forms that were newly added to the eOPF Core Forms List
Agency-specific forms that were mapped to standard Core or DG forms
Forms that were removed or reclassified
These updates help standardize records across agencies and improve transfers between agencies and NPRC.
Why this matters?
Using the correct forms and indexing:
Prevents transfer errors during agency moves or separations
Ensures documents are filed on the correct folder side
Reduces rework, audit findings, and Help Desk issues
Supports compliance with the Guide to Personnel Recordkeeping (GPR)
Some updates may automatically reindex existing documents, so understanding the changes helps you answer employee questions and identify expected system behavior.
