The federal forms for Entrance on Duty (EOD) are listed in Appendix D of the HR LOB EOD CONOPS, published by HR LOB in 2007 and updated by OPM in 2021. Agencies can request to add forms beyond the standard list by contacting the eOPF PMO for approval. Section 2.0 Getting Started of the eOPF Entrance on Duty (EOD) Requirement and System Certification guidance indicates that if the agency wants to include more than the standard EOD forms identified in Section 3.1 in their EOD system, the agency eOPF POC should contact the eOPF PMO to facilitate form approval and requirements. If the forms are approved for submission to eOPF, the eOPF PMO will facilitate addition to the OPM Master Forms List.
